CORE Initiative’s Peer Support Program connects people living with diabetes with trained peer facilitators who understand the challenges of managing diabetes firsthand. Through group-based support, education, and meaningful connection, participants gain practical tools, encouragement, and a community where they can share experiences without judgment.

Our peer support groups focus on the whole person—not just diabetes management. Participants discuss healthy lifestyle changes, emotional well-being, navigating healthcare, overcoming barriers, and building confidence in managing their health. Groups are designed to be welcoming, culturally responsive, and accessible to diverse communities, with in-person, virtual, Spanish-speaking, and specialized group options available.

At CORE Initiative, we believe people are often empowered most when they learn with and from one another. Our peer support model creates a space where lived experience becomes a source of knowledge, encouragement, and lasting change.
